When calling for an appointment, insurance information will be requested. We ask that patients arrive 10 - 15 minutes prior to their scheduled appointment to complete registration and to confirm their insurance information.
We also ask that copies of test results, reports, and any CT scans necessary for review are brought with or delivered to the office prior to the scheduled appointment time. Reports can be faxed to the office at (833) 760-3767.
Patients should be able to obtain copies of their radiology studies (X-rays, CT scans) and our office can assist in obtaining records from hospitals and other physicians' offices.
If the office is closed, messages can be left on the office voice mail for retrieval the next business day. This is most appropriate for test result requests, appointment cancellations and requests for appointments.
